Mandatory COVID-19 Screening

Please fill out this quick survey prior to your visit to help everyone stay safe and healthy!

1. Do you have a fever?
2. Do you have any of the following symptoms?
3. Have you traveled or have had close contact with any who has traveled in the past 14 days?
4. Have you had close contact with a confirmed or suspected case of COVID-19?
5. Did you wear the recommended/required PPE when performing the type of duties you were performing in close proximity to a COVID-19 case?

If you have answered no to questions 1, 3, 4 and no signs/symptoms of COVID-19 your child may proceed with participating in our programs. If you have answered yes 1, 3, 4 and have checked off signs and symptoms you may need to reschedule. If you have answered yes to question 4 and 5 your child may proceed with participating in our programs.

I understand that Young Moviemakers has taken appropriate steps to minimize my child's risk of exposure to COVID-19. However, it is understood that, although minimal, there is a risk that my child may be exposed to COVID-19 and Young Moviemakers is not liable in this circumstance.
