Young Moviemakers x Collingwood Media Release

Dear Parent/Caregiver,

Due to the nature of our programs, all of our films are uploaded onto our Young Moviemakers YouTube account. Only first names are included in our credits on YouTube; however, some festival entries and other exceptions may ask for last names. We understand that there may be some concerns regarding releasing images onto YouTube but unfortunately, this is the best way we can share and distribute films to participants. If Young Moviemakers does not receive media consent for each participant, please be aware that we must find a way to hide your child’s images on camera by avoiding filming them directly or putting them in a mask.

THE UNDERSIGNED HEREBY GRANTS Young Moviemakers and their partners, the absolute right and permission to use and re-use, in whole or in part, the name, voice, images, likeness and artworks of the person named in this release. Such videos and artwork produced or provided for use at any point during the course of a Young Moviemakers program, are granted use in any and all media worldwide in perpetuity for any and all legal purposes, including advertising, promotion, records, public relations or internet programs, without payment or notice to the undersigned.
THE UNDERSIGNED HEREBY CONSENTS that Young Moviemakers is the owner of all intellectual property rights to the artworks produced during this program, the videos and all film, footage, recording, soundtracks, images and reproductive product (including any digital reproductions) arising out of the videos and artwork (the “Intellectual Property), and the undersigned hereby unconditionally waves any and all personal and proprietary right, including all moral rights, that the undersigned may have in the Intellectual Property.
THE UNDERSIGNED HEREBY UNDERSTANDS that any such materials (video, audio, film or any other media) will be used only for the purpose of promoting the program and its participants, at festivals, screenings, broadcast, program promotions and distributions. It is also understood that any such materials will be used with the highest integrity and discretion, with the intent to communicate responsibly and ethically, the subject matter contained therein.

Waiver of Liability Agreement

I am aware that my child will be participating in filmmaking, theatre games and other artistic activities through Young Moviemakers. (the “Activities”). I am aware that participation in the Activities involves risks, dangers and hazards including, but not limited to: slips and falls; equipment mishandling; prop mishandling; environmental hazards; and negligence of other persons.

I have reviewed with my child the importance of being safe while participating in the Activities including maintaining a respectful on-set etiquette, following instructions as it relates to equipment and prop handling, being respectful to all staff and other participants and following all rules, guidelines and directions from Young Moviemakers instructors and other staff.

My child does not have any health conditions which would prevent him/her from participating in the Activities.

I give my permission to Young Moviemakers to provide medical care or arrange for medical care to be provided to my child during the Activities, should they deem it appropriate. I understand and agree that I am fully responsible for all costs of medical care from the Activities. I will reimburse Young Moviemakers for any and all such costs.

I hereby confirm that I have read and understood this document prior to signing it and agree that this document will be binding upon my, my heirs, executors and assigns.
